NON-OCCUPATIONAL SHORT TERM DISABILITY INSURANCE
Eligibility:All active regular and regular part time employees of Utah Power/PacifiCorp who are members of IBEW Local 57 who work at least twenty (20) hours per week.
Benefits Design: Eighth (8th) Day Sickness Eighth (8th) Day Accident
Maximum Benefit Period: Up to thirteen (13) weeks.
Definition of Disability: The inability, due to sickness or injury, to perform the material and substantial duties of one's job.

How premiums are paid: By convenient payroll deduction.
Waiver of premium: For your period of qualified disability, no premiums are due.

Short-term disability insurance is a non-occupational insurance that provides a weekly benefit of $175 to $350 depending on the insured's classification for a maximum of thirteen (13) weeks of disability. The premiums are based on member's classification and their age. The program has been in effect since 1989, and many of our members have benefited from it.

As a participant in the Local Union's Short Term Disability Program, members are eligible to receive benefits if they are under the care of a physician and the disability is not the result of a work-related illness or accident. Benefits begin on the eighth consecutive day of the disability and continue for thirteen weeks or when the members return to work, whichever occurs first.

This insurance is available to Union members only who work for Utah Power/PacifiCorp and the member pays the insurance premiums through Utah Power payroll deduction. If Union membership is dropped, eligibility for this insurance is also dropped. When a person joins the Union for the first time, this policy is offered under open enrollment. A person opting to not sign up for STD at the time of joining the Union will not be covered by the open enrollment clause. "Evidence of Insurability" form will have to be filled out and submitted along with your application form at the time of late enrollment, and a physical exam may or may not be required.
